We request all grant recipients to submit a report to the Connelly Foundation by the deadline indicated in the award letter. This report will be used to assess both the outcome of your project and the impact of our funding.
Organizations that have received a grant from Connelly Foundation are required to submit a final report using the DVG Common Report Form before being considered for a new grant. Final reports need to be submitted under separate cover. The DVG Common Report form is located at the end of the DVG Application Form at the below link.
