We request all grant recipients to submit a report to the Connelly Foundation by the deadline indicated in the award letter. This report will be used to evaluate both the outcome of all projects, plus the potential for the organization to receive future funding.
The report need not be long, but it should be be clear, concise and complete. Please include the following elements:
- Name of organization, award date, grant amount, contact name and phone number.
- Self evaluation of accomplishments as compared to stated goals.
- Perceived or anticipated benefits to: the targeted population, the organization, and others if possible.
- Any problems, difficulties or delays encountered. (e.g. Did the project meet the proposed timeline?)
- Pertinent financial information such as specific expenditures from this grant, total program expenditures and fund raising success.
- Comparison of projected versus actual budget for grant year.
- Brief mention of any significant organizational or personnel changes which may have occurred during the grant year.
